Thank you for registering for LYS!  By printing, filling out this simple form and mailing it to us with your tuition deposit we will have the information we need for you or your child to attend LYS this summer.  Please note that information about tuition, what to pack, etc. can be found in a password protected place on our web site after you fill out this form and MAIL IT TO: Louisiana Youth Seminar; 
P.O. Box 14241; Baton Rouge, LA 70898. The username and password will be emailed to you after your registration form is received - there you can download helpful information for registrants of LYS!
